Video Request Instructions
Thank you for your request for a copy of video from a Mister Car Wash location. These instructions serve to address two different types of requests: (1) copy of video, and (2) video preservation.
COPY OF VIDEO REQUESTS:
a) STEP ONE. VERY IMPORTANT: A subpoena, warrant, or court order IS REQUIRED. If you do not have a subpoena, warrant, or court order, STOP NOW and do not proceed any further with your copy of video request.
i) If you are a customer, customer attorney, or insurance company, be advised that we do not provide video directly or indirectly to customers absent a statutory or other legal requirement.
ii) If you are a Mister Car Wash customer with a vehicle damage claim, call Mister Car Wash Customer Service at 844-466-2468 for further assistance with your claim.
b) STEP TWO. QUESTIONNAIRE OF REQUIRED INFORMATION: If you have met the requirements of STEP ONE above, then proceed to the Questionnaire of Required Information in SECTION III

YOUR EMAIL REQUEST TO VIDEOREQUEST@MISTERCARWASH DOES NOT CREATE A LEGAL OBLIGATION FOR MISTER CAR WASH TO PRESERVE EVIDENCE. IF YOU WANT TO CREATE A LEGAL OBLIGATION TO PRESERVE EVIDENCE, SEND A SUBPOENA, COURT ORDER, OR LETTER ON FIRM LETTERHEAD CITING THE APPLICABLE STATUTE OR RULE, VIA CERTIFIED MAIL TO: GENERAL COUNSEL, 222 E. 5TH STREET, TUCSON, AZ 85705. YOUR SUBPOENA, COURT ORDER, OR LETTER MUST CONTAIN THE QUESTIONNAIRE OF REQUIRED INFORMATION (SEE IN SECTION III BELOW). THERE IS NO WARRANTY OR ASSURANCE THAT THE VIDEO DESIRED IS CURRENTLY AVAILABLE OR WILL BE AVAILABLE AT THE TIME YOUR COMPLETED REQUEST IS RECEIVED.
i) If you are a customer, customer attorney or insurance company, be advised that we do not provide video directly or indirectly to customers absent a statutory or other legal requirement. Any request for preservation of evidence must include the Required Information (see in SECTION III below). If you are a Mister Car Wash customer with a vehicle damage claim, call Mister Car Wash Customer Service at 844-466-2468 for further assistance with your claim.
ii) If you are a law enforcement officer/agency or an attorney representing someone other than a Mister Car Wash customer, please send a subpoena or court order for the requested video including the REQUIRED INFORMATION below in SECTION III, via USPS Certified Mail to: General Counsel, 222 W. 5th Street, Tucson, AZ 85705.
iii) If you are an insurance company representing a customer with a pending subrogation claim against Mister Car Wash, please reply with an executed letter on your firm letterhead including the Required Information below in SECTION III. Upon receipt of the Required Information, your request will be entered into the queue, and we will review and do our best to reply within 3 to 4 weeks of receipt.

Statement: By submitting this Questionnaire, the person requesting video from Mister Car Wash agrees as follows: the requested video will be used only in connection with the referenced investigation or lawsuit, will not be generally distributed to the public, and will not be posted to any social media website; if requested by the other side of the investigation or lawsuit (if applicable), it will be shared with them; the video will not be edited, altered, or otherwise modified without expressly indicating that it has been edited, altered, or otherwise modified, and an original unedited, unaltered, and unmodified copy of the requested video will be retained in the event any editing, altering, or modifying takes place.
